About this study
This trial is testing whether a simple cognitive assessment called the 5-Cog paradigm, used with a clinical decision tool, helps doctors find people who need help for memory problems (cognitive impairment) more often than usual care does. The goal is to see if this approach can lead to better diagnosis and treatment of dementia in older adults, especially those from underserved communities.
